APPLICATIONS RENEW EACH SCHOOL YEAR OR ONCE PER CALENDAR YEAR.

PLEASE CHECK WITH OFFICE ON WHAT IS NEEDED.

Coaches AND assistant coaches required paperwork:
Volunteer app, background check (18+), concussion training, code of conduct, youth safety policy

Prospective Board of Directors:
Volunteer app to start; background check if elected to the board

Referees/time keepers, book keepers:
Volunteer app, background check (18+), concussion training
If under 18 a parent/guardian will need to sign the application as well

Volunteer app:
This is good for the current school year and will renew each fall.

Background checks: 

This is renewed once per calendar year. 

Concussion training:
This is renewed once per calendar year.

Youth Safety Policy:
This is renewed once per calendar year.

REFUNDS: Anyone requesting a refund is REQUIRED to fill out the form.

SCHOLARSHIPS: Anyone requesting a scholarship will be
​REQUIRED to fill out the scholarship request form.
